Masters-level Social Work Internship with Intensive Care Coordination Program
15-20 hours per week for both Fall and Spring Semesters (intern is also welcome to start during the summer of 2021 with permission from their school, and must still be available for the Fall and Spring semesters as well)
Position Summary: The purpose of the ICC program is to help clients who are discharging from higher levels of care, by connecting them with necessary services and supports to maintain their stability post-discharge. In this position, the intern will create collaborative relationships with numerous different entities both inside and outside of the organization. This position is likely to remain at least partially remote even after COVID; however, there is office space at the Centre building that's shared by the ICC team.
Intern Responsibilities: Intern will be part of the Intensive Care Coordination team at SummitStone Health Partners assisting in care coordination with local, regional, and state psychiatric hospitals. Opportunities will include:
Requirements: Entering final year of MSW program in an accredited college or university; Experience with Microsoft Suite (Word, Outlook, Excel, etc.).
Preferred: Familiarity with resources and supports within Larimer County; Experience with Trauma-Informed Care; Experience with acute mental health disorders and substance abuse disorders.
