Position: Case Manager, Crisis/Stabilization Services
Full Time & PRN available: 36-40 hours per week; 12-hour shifts (days, nights)
Responsibilities: The case manager provides services to individuals experiencing a behavioral health crisis. Duties include milieu management, brief counseling interventions, psycho-education and activities groups, and treatment planning within a Crisis Stabilization Unit setting. The position interacts extensively with a large interdisciplinary team including RN's, Psychiatrists, Nurse Practitioners, Mental Health Therapists, and Psychologists.
Required: Bachelor's degree in behavioral science; Work experience with people severe and persistent mental illness/substance abuse issues or individuals with similar human-service needs; experience with Word, Excel, and Outlook. Valid Colorado driver's license with ability to be insured by SHP.
Preferred: Bilingual/Bicultural; Trauma Informed Care experience. CAC.
