Position: ACT, Case Manager
Location: ACT (Assertive Community Treatment)/CDDT (Community Dual Disorder Treatment)
Position Status: 20 hours per week
Position Summary: This is a part-time position - 0.5 FTE Case manager for the ACT/CDDT teams. Case management will include intensive outreach provided almost wholly in the community. Responsible to carry out rehabilitation and support function and assist in treatment; provides resource acquisition, education, and support; and crisis intervention under the clinical supervision of staff with professional degrees. Examples of activities include delivering medication, facilitating an educational group, accompanying clients to doctor's appointments, filling out applications, intervening with landlords, and coordinating care with other service providers.
Wage Range- Between $17.59/HR and $24.62/HR.
Required: Bachelor's degree in behavioral science or 1 year clinical experience at a mental health agency; Work experience with people severe and persistent mental illness/substance abuse issues or individuals with similar human-services needs; strong organizational skills experience with Word, Excel and Outlook. Valid Colorado driver's license with ability to be insured by SHP.
Preferred: Bilingual/Bicultural; Trauma Informed Care experience. CAS.
